Reports to: Chief Program Officer
Summary/Objective
The Referral/Intake Coordinator acts as the liaison between the referral source and the organization to ensure that referrals are processed in a timely manner and ensures a seamless transition into residential services.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions
- Establishes and maintains relationships with referral sources by receiving and processing intake information for potential residential services clientele;
- Facilitates intake process by procuring all relevant clinical and financial information necessary to determine eligibility;
- Performs tours for the individual, families, and guardians;
- Provides referral source notification of acceptance or denial of referral to individuals and other interested parties
- Provides service coordination entity with needed information for Utilization Review
- Tracks all referral information in database
- Enters individual information into online case management portal
- Sets up client new informational binders
- Coordinates internal moves and provides direction on roommate matches
- Facilitates weekly moves and transition meetings

Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands
This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand as necessary. Also requires walking up and down stairs as needed.
Position Type and Expected Hours of Work
This is a full-time position. Days and hours of work are: Monday through Thursday 8:30am to 5:00pm.
Travel
No travel is expected for this position
Required Education and Experience
- High school diploma/GED
- Proficient computer skills and in-depth knowledge of relevant software such as MS Office Suite, especially excel
- Knowledge of standard office administrative practices and procedures
Preferred Education and Experience
- Bachelor’s degree an advantage
