JOB SUMMARY: The Patient Navigator will be under the direct supervision of the Health Homes Supervisor and will be responsible for outreach and enrollment of potential Health Homes clients. The Patient Navigator will deliver largely field-based services to Health Homes clients. You as a Patient Navigator, are responsible for cultural humility and working collaboratively with individuals to receive the necessary care they need. As a result, you get to help individuals stay safe and healthy.
DUTIES AND RESPONSIBILITIES:
- Outreach to all potential Health Homes Members within ten (10) business days of assignment
- Conduct outreach in the form of; phone calls, emails, letters, text messages, and home visits to locate clients and introduce them to Health Home services
- Document all outreach attempts within the internal and State Systems
- Creation of member charts within the internal system
- Create and maintain an updated client spreadsheet
- Assist Care Manager with re-engaging clients as needed
- Provide escort services to clients as needed
- Take part in agency committees as assigned
- Attend in-house and off-site trainings
- Perform all job duties in alignment with the Department Of Health, Health Home, and BAC standards
- Other duties as assigned and required by the program and BAC
KNOWLEDGE, SKILLS AND ABILITIES:
- Ability to travel 75% for client outreach and home visits.
- Comfortable traveling where clients stay (shelters, SRO’s, private apartments, etc.) for outreach and home visits to facilitate patient access to client services.
- Novice Health Home guidelines and procedures
- Intermediate knowledge of HIV/AIDS prevention, transmission, treatment, and co-occurring concerns such as substance use.
- Competent proficiency with Microsoft Office
- Proficient in motivational interviewing skills
- Competent case management assessment skills
- Strong interpersonal communications skills and the ability to empathize with, relate to, and effectively work with peers and clients.
- Possesses excellent judgment and the ability to maintain professional relationships and boundaries when working with clients and peers
- Strong verbal and written communication skills
- Competency in navigating multiple databases, inputting, and uploading information
- Bilingual Spanish/ French/ Creole a plus.
EDUCATION:
- MINIMUM: High school diploma or equivalent.
