North Dakota State Hospital - Jamestown, ND - Mental Illness Case Manager II - Residential Services

The North Dakota State Hospital (NDSH), located in Jamestown, is a specialized psychiatric hospital that serves the whole state. Services focus on psychiatric rehabilitation, residential substance use disorders treatment, and residential treatment for sexually dangerous individuals.

Be part of the solution to the behavioral health crisis. As a team member for ND Department of Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover.

As a Mental Illness Case Manager II, you will ensure successful admission, treatment, and discharge planning for clients admitted to the North Dakota State Hospital Residential Services by providing screening, documentation, intensive case management, as well as collaboration and documentation with the Department of Corrections and other entities, and treatment team collaboration. On a daily basis you will perform basic assessments of clients’ current functioning and needs, develop a care plan to address those needs, and refer and link to needed community resources, supports, and services identified in the care plan. In addition, you will monitor and follow up care plans to ensure they are effectively implemented and adequately addressing the needs of the clients.

To be considered for this position, you are required to have a bachelor's degree in social work, psychology, counseling, nursing, occupational therapy, vocational rehabilitation, therapeutic recreation, or human resource management (human service track), and two years of experience working with special population groups in a direct care setting; OR a master's degree in one of the fields listed.

Preference will be given to applicants with education and/or experience in Social Work.
