The housing Programs provide permanent and temporary affordable housing to individuals living with mental health concerns. The temporary housing programs function as a step towards permanent and independent living.
Overview
The PROS/Housing Registered Nurse (RN) is responsible for health screenings and the assessment and monitoring of programs participants' psychiatric, medical and substance use status. In addition, the RN is responsible for providing education to program participants and to staff members on key issues as they relate to psychiatric, medical and/or substance issues. They assist programs participants in connecting with a PCP and other medical providers and liaise with them as needed. The RN has discretion for exercising independent, professional judgment in carrying out the details of the job.

- The weekly set schedule is designed to allow the provision of services in the community in multiple programs, including a few housing sites and a site-based PROS program (all located in the EV/LES and 1 site in Brooklyn.)
- Completing Health Assessments, including screening for metabolic syndrome, diabetes and hypertension as specified by program's protocol.
- Assist with monitoring and coordination of program participants' medical, psychiatric, and substance use treatment, in conjunction with various internal and external service providers.
- Assist programs' participants with medication management and symptoms monitoring.
- Maintain schedule and administer IM and blood tests scheduled/required.
- Provide advocacy for program participants with their care providers.
- Provide education (1:1 and/ groups) on health, psychiatric, and substance use issues, including medication use and side effects to programs' participants and programs' staff.
- Assures that all requisite documentation and charting is completed in an accurate and timely manner in accordance with appropriate programs' protocol and regulations.
- Monitors psychiatric medications and lab work and works closely with presenting program prescriber (when applicable) to ensure medication concerns are addressed.
- Consult with outside health and psychiatric care providers on health and psychiatric care issue.
- Perform other duties as assigned.
- Completed an accredited course of training for nurses, a NYS RN License.
- A minimum of two years of nursing experience in public health, mental health, or psychiatric nursing in a position requiring the provision of direct service to participants in the community preferred.
- Must be fingerprinted and cleared by the Office of Mental Health (OMH).
- Commitment to recovery oriented practice.
- Be skilled in conflict mediation/negotiation and have an assertive approach to problem solving.
- Excellent oral and written communication skills.
- Ability to utilize various computer programs, specifically Microsoft Word and Excel.
- Be creative and flexible.
- Show initiative and be responsible for follow through.
- Ability to maintain confidential information, as related to position.
- Ability to work independently and as part of a team.
- Bilingual Spanish-speaking preferred
