Job Title: 249 LPN/248 LPNII
Provides quality care that is dependent on physician order or directed by the RN. Delivers nursing interventions based on professional judgment in response to individualized patient needs. Promotes restorative health and/or supportive care using the nursing process to assess, plan, implement and evaluate nursing care.

Job Components

C. Core Roles and Responsibilities
- Assesses patient needs and gives appropriate care for patient age newborn to 18 years of age.
- Identifies patient before treatment by name, birthdate, parent
- Assesses pain and provides pain management
- Performs call backs in a timely manner, recommends office visits or calls in prescriptions, give assurance to caregiver.
- Questions patient/parents about reason for visit and documents appropriately
- Takes vital signs and documents in EMR.
- Functions as a member of healthcare team.
- Demonstrates appropriate communication skills with physicians and other team members
- Accepts additional assignments and assists other staff members willingly.
- Insures patient care is accurately documented
- Completes assignments in a timely manner
- Demonstrates initiative as a self-starter
- Demonstrates knowledge of nursing care.
- Keeps up-to-date on new medications
- Demonstrates proper medication administration when ordered.
- Gives injection at appropriate site and at appropriate time and does when ordered
- Demonstrates proper lab techniques
- Demonstrates ability to assist physician with procedures
- Follows procedures to obtain consent to treat and/or give immunizations
- Responsible for preparing for well checks and EPSDT Medicaid screens.
- Does age appropriate work up for screens and well check including hearing and vision.
- Has knowledge of safety with sharps and demonstrates this in giving shoots.
- Does lead testing when patient is at specific age
- Adjusts testing procedure according to age and size of patient.
- Assesses need for additional safety measures.
- Assesses signs and symptoms of abuse of neglect, reports/documents same.
- Demonstrates actions which create a positive experience for the patient.
- Maintains patient confidentiality, conforms to safety rules
- Makes eye contact, smiles, greets each patient/parent
- Communicates processes, procedures, explains delays
- Listens actively and responds promptly to expressed needs
- Personalizes communication and approach, allows choices when possible.
- Teamwork-Employee: promotes positive teamwork with co-workers.
- Listen and make active attempt to understand co-workers.
- Deals with co-workers in an unbiased manner
- Used self-control and demonstrates flexibility
- Verbally supports policies and decisions.
