Hours: M-F 8am-4:30pm

Summary:

• Self-directed role: each home care nurse has their panel of patients, who they’re responsible for covering. This includes 2x/year assessments as well as any ongoing care (medication administration, wound care, BP/wellness checks). This role requires a nurse who is independent, skilled, and highly organized.
• You will be covering Quincy, South Boston, and Mattapan area - will be visiting Harbor Health's patients that are housed in assisted living facilities and nursing homes in those three towns/areas.

Requirements:

• Licensed RN in MA
• Active BLS
• Must have VNA, homecare, skilled nursing experience
• Must be comfortable with going to patient’s homes (within the assisted living facility)
• Wound care experience
• Must be able to self-organize and work independently
• Two (2) or more years’ experience in a home care, professional or business environment.
• One year of experience working with geriatric patients require

Responsibilities:

• Rounding on all necessary patients – traveling to their homes on site
• Wound care
• Blood pressure checks
• Preventable assessments

• Performs routine clinical nursing duties, including history taking, vital signs, administration of medications, dressing changes and other nursing procedures under the verbal or written order of a provider as prescribed by law, written clinical policies and within the limits of his/her own license and training.
• Responsible for Case Managing home based clients through communication with providers, center nursing staff, specialists, Clinical Nurse Manager, Clinical Director and all other ESP staff members involved in participant care.
• Completes nursing assessment and collaborates in the development of a comprehensive plan of care, as required by the Patient Care Team.
• Triages patients regarding illness and medical therapy and advises the patient/caregiver of the most appropriate action in consultation with a provider whenever necessary.
• Interacts with client and family members in a manner that adheres to the highest standards of care and professionalism.
• Builds trust and fosters smooth relationships with clients and their families.
• Promotes patient confidentiality and right to privacy and cooperates with Medical Records staff to ensure effective return and retrieval for patient records.
• Performs other related duties as assigned.

Pay: $40.00 - $45.00 per hour
