Clinical Nurse II – PRN Triage Nurse - Duke Asthma, Allergy and Airway Center/Pulmonary

We are seeking a TRIAGE NURSE to be an integral member of our Asthma, Allergy, and Airway Center healthcare team. The position will manage this patient population via telehealth in order to coordinate care with providers. The Triage Nurse position is based in Durham, NC at a non-clinical office site. This position interacts with patients via telephone and secure messaging vs direct patient contact. The position is located at 4825 Creekstone Drive, Durham, NC. Hours are from 8:00 am to 5:00 pm Monday through Friday. No weekends or Duke designated holidays. Once fully on boarded, this work can be done as a hybrid model. This means a rotating schedule of working remotely and from the office.

JOB DUTIES:

  • Provide nursing triage, assessment and education to patients via telephone interactions and secure messaging.
  • Manage patient needs including medication management, post procedure instructions and care coordination.
  • Serve as a liaison to patients, family members and referring providers with guidance from pulmonary providers.
  • Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate appropriately nursing actions taken and patient responses.
  • Delegate tasks and direct the activities of unlicensed care providers (schedulers, Medical Assistants and Staff Assistants).
  • Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings.
  • Participate in performance improvement and patient experience activities.
  • Perform other related duties incidental to the work described herein.


REQUIRED QUALIFICATIONS:

  • Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
  • All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date.


KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of scope of the registered nurse, licensed practical nurse and all unlicensed assistive personnel
  • Knowledge of and appropriate application of the nursing process
  • Knowledge of professional theory, practice and procedures
  • Ability to assess nursing needs of acute and chronically ill patients and their families. Ability to independently seek out resources and work collaboratively
  • Ability to establish and maintain effective working relationships
  • Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others
  • Ability to teach patients and families in accordance with the nursing plan of care
  • Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up
  • Ability to record activities, document assessments, plan of care, interventions, evaluation and re-evaluation of patient status
  • Typing a minimum of 45 words per minute
  • Experience with Epic documentation platform
