CCM (Chronic Care Management) Coordinator
Job description
Do you have experience in Chronic Care Management? Alteas Health is seeking an experienced Chronic Care Management Coordinator to oversee the daily operations of CCM, specializing in cardiac and pulmonary services, while working under the guidance of Clinical Operations Managers. The Coordinator will provide top-quality administrative support to ensure the best care for patients. This includes monitoring eligible patients who have given treatment consent, reviewing daily vitals, lab results, medications, and weights, and using defined clinical algorithms to identify critical values. The Coordinator will collaborate with Medical Assistants to ensure accurate documentation of patient minutes for proper payment and claims.
What are the must haves (aka minimum requirements)
- Certified Medical Assistant or equivalent
- Proficiency in MS Office, EMR software, and PCC
- Familiarity with Chronic Care Management programs
- Knowledge of Medical Terminology
- Provide professional and appropriate communication to staff and advanced practice providers.
- Typing speed of 50 wpm.
- Other requirements that are nice to have but not required:
- 2+ years of Cardiology and/or Pulmonology office experience
- 2+ years of long-term care experience
Tell me about the job duties (aka key responsibilities but not all encompassing)
- **This is a hybrid in office position.** Office is located in Highland Park, IL.
- This position does not involve direct patient care.
- Assist the clinical managers in the growth of the CCM program.
- Identify and update eligible CCM patients.
- Continually communicate with advanced practice providers regarding patients who are eligible for CCM enrollment.
- Ensure that provider documentation contains verbal consent and an accurate care plan needed for CCM enrollment.
- Report to the Clinical Operations Manager any potential provider issues or retraining needed.
- Work closely with the Medical Assistants to ensure patient data that falls outside of the set parameters is communicated to the assigned provider.
- Oversee the Clinical Medical Assistants and be a resource to them as needed.
- Troubleshoot system glitches and utilize appropriate resources.
- Manage and document appropriate minutes of care for payment and claims.
- Understand, report, and assist with care plan management for high-risk diagnoses and chronic conditions.
- Other assignments as needed for CCM and patient care management.
